কর্কট রোগ তথা ক্যান্সার – নিরাময়ে সমস্যা শরীরে না সমাজ-ব্যবস্থায়?

যে সমস্যাটি বিষয়ে আজকের অবতারণা সে সম্পর্কে শিরোনাম থেকেই সহজে অনুমেয় যে, প্রসঙ্গটি নিতান্তই সহজ তো নয়ই বরঞ্চ যৎপরোনাস্তি বিতর্কিত – অনন্তর, পাঠককূলের কাছে অনুরোধ, বর্তমান আলোচনা ও পর্যালোচনা সযত্নে অনুধাবন করুন।
বিশ্ব স্বাস্থ্য সংস্থার ০৩-০২-২০২৫ তারিখের প্রতিবেদন অনুসারে গুরুত্বপূর্ণ তথ্য – (১) ক্যান্সার বিশ্বব্যাপী মৃত্যুর একটি প্রধান কারণ, ২০২০ সালে প্রায় ১ কোটি মৃত্যু, অর্থাৎ প্রতি ছয়জনের মধ্যে প্রায় একজন; (২) সবচেয়ে সাধারণ ক্যান্সার হল স্তন, ফুসফুস, কোলন ও মলদ্বার এবং প্রোস্টেট ক্যান্সার; (৩) ক্যান্সারে মৃত্যুর প্রায় এক-তৃতীয়াংশ তামাক ব্যবহার, উচ্চ বডি মাস ইনডেক্স, মদ্য পান, কম ফল এবং শাক-সব্জি গ্রহণ এবং শারীরিক কার্যকলাপের অভাবের কারণে হয়, এছাড়াও, বায়ু দূষণ ফুসফুসের ক্যান্সারের জন্য একটি গুরুত্বপূর্ণ কারণ; (৪) নিম্ন এবং নিম্ন-মধ্যম আয়ের দেশগুলিতে প্রায় ৩০% ক্যান্সারের জন্য ক্যান্সার সৃষ্টিকারী সংক্রমণ, যেমন হিউম্যান প্যাপিলোমাভাইরাস (HPV) ও হেপাটাইটিস দায়ী এবং (৫) প্রাথমিকভাবে সনাক্তকরণ ও কার্যকরভাবে চিকিৎসা করা হলে অনেক ক্যান্সার নিরাময় করা সম্ভব (তথ্যসূত্র [১])। প্রসঙ্গতঃ উল্লেখ্য যে, এই সংস্থার অধীনে International Agency for Research on Cancer ২০২২ সালের জন্য ভারতে ক্যান্সার প্রবণতা তথা ঝুঁকির উপর একটি গুরুত্বপূর্ণ প্রতিবেদনে আক্রান্ত-অঙ্গ এবং লিঙ্গভেদ ও বয়স বিচার পূর্বক বিস্তারিত পরিসংখ্যান ০৮-০২-২০২৪ তারিখে প্রকাশ করেছে (তথ্যসূত্র [২])।
Indian Journal of Medical Research পত্রিকায় ১১-০৩-২০২৩ তারিখে প্রকাশিত (তথ্যসূত্র [৩]) একটি গবেষণা প্রবন্ধে উল্লেখ যে, ২০২২ সালে ভারতে ক্যান্সারের আনুমানিক ঘটনা ১৪,৬১,৪২৭ (প্রতি ১০০,০০০ জনে ১০০.৪) পাওয়া গেছে। ভারতে, প্রতি নয় জনের মধ্যে একজনের জীবদ্দশায় ক্যান্সার হওয়ার সম্ভাবনা রয়েছে। পুরুষ এবং মহিলাদের মধ্যে যথাক্রমে ফুসফুস এবং স্তন ক্যান্সার প্রধান ছিল। শৈশবে (০-১৪ বছর) ক্যান্সারের মধ্যে লিম্ফয়েড লিউকেমিয়া (ছেলেরা: ২৯.২% এবং মেয়েরা: ২৪.২%) ছিল শীর্ষস্থানে। ২০২০ সালের তুলনায় ২০২৫ সালে ক্যান্সারের ঘটনা ১২.৮ শতাংশ বৃদ্ধি পাবে বলে অনুমান করা হচ্ছে। ঐ ভারতীয় সংস্থাটি দ্বারা সংগৃহিত ও প্রকৃয়াকৃত এই পরিসংখ্যান ২০২৩ সালের মার্চ মাসেই প্রকাশিত হয়েছিল (তথ্যসূত্র [৪])।
যাক, কিঞ্চিৎ পরিসংখ্যান পরিবেশনপূর্বক বর্তমান প্রসঙ্গ তথা বাস্তব পরিস্থিতি বিশ্লেষণ করে একটি সুনিদিষ্ট সমাধানের লক্ষ্যে আগাতে যৌথভাবে আমরা চেষ্টা করি। আমরা হয়তো সকলেই জানি যে, কর্কট রোগ তথা ক্যান্সার সাধারণভাবে জীবানুঘটিত নয় এবং সেকারণে ছোঁয়াচে নয় । জীবদেহের অতি ক্ষুদ্র অনেকাংশেই স্বয়ংসম্পূর্ণ জীবন্ত একক সত্তা হল কোষ, এরূপ কোটি কোটি কোষ নিয়ে গঠিত জীবদেহ ও তার অঙ্গ-প্রত্যঙ্গ; দেহের কোন বিশেষ অঙ্গ বা তার একাংশের কিছু অস্বাভাবিক কোষের অতি দ্রুত সংখ্যা বৃদ্ধির ফলে স্বাভাবিক কারণে স্থানাভাবে হয় দুষ্ট টিউমার বা তা ফেটে গিয়ে দুষ্ট ক্ষত সৃষ্টি করে এবং ক্যান্সার নামক ব্যাধির সূচনা করে। যদিও সমস্ত জীবকুলের অনন্তর আমাদেরও স্বাভাবিক জৈবিক প্রক্রিয়ায় প্রতিদিন অসংখ্য কোষের মৃত্যু ও এই ক্ষয়-পূরণ ও বিশেষতঃ কমবয়সিদের ক্ষেত্রে বৃদ্ধিসাধনের জন্য নূতন কোষ সৃষ্টিও হয়। কিন্তু ঐ ক্যান্সার কোষগুলির মৃত্যুর তুলনায় জন্ম অত্যধিক বেশি হারে ঘটে – সুতরাং ঐ ক্ষত যেমন উত্তরোত্তর বৃদ্ধি পায় তেমনই অক্সিজেন ও খাবার ঐ দুষ্ট কোষগুলির পুষ্টি ও তাদের অপত্য কোষ সৃষ্টি-প্রক্রিয়ায় অনুচিত অগ্রাধিকার দাবিপূর্বক ব্যবহার হওয়ার কারণে স্বাভাবিক কোষের পুষ্টি ও ক্ষয়পূরণ ও বৃদ্ধিসাধন ব্যাহত হতে থাকে! সুতরাং, (১) জীবনযাত্রা ও খাদ্যাভ্যাস এমনই হওয়া উচিৎ যাতে ক্যান্সারের প্রবণতা তথা ঝুঁকি কমান যায় এবং (২) রোগটি প্রাথমিক স্তরে বা প্রাকট্য অতিমাত্রায় বৃদ্ধি পাওয়ার আগে নিরাময়ের ব্যবস্থা নেওয়া দরকার – অর্থাৎ, যথাযথ চিকিৎসা ব্যবস্থার সহায়তায় (ক) সরাসরি ঐ দুষ্ট কোষগুলিকেই মেরে ফেলা উচিৎ ও/বা (খ) তাদের বংশবৃদ্ধির হার অতি দ্রুত কমিয়ে শূন্যে এনে দেওয়া দরকার। এই পরামর্শ নিছক শিক্ষামূলক আলোচনার স্বার্থে মেনে নিতে কারও আপত্তি থাকার সঙ্গত কারণ নাই এবং পরিত্রাণের উপায়গুলিও নিতান্তই সোজা ও সহজ বলে মনে করা যেতে পারে – কিন্তু, সমস্যা বিশেষতঃ সংশ্লিষ্ট মনোস্তাত্তিক ও সামাজিক সমস্যাসমূহ এমনই গভীরে যে সাম্প্রতিককালে আমার তিনজন পরমাত্মীয়ের বিশেষতঃ অতি সম্প্রতি আমার ঠিক পরের বোনের এই রোগের কবলে পড়ে মর্মান্তিক (অর্থ-সঙ্কটে নয়) মৃত্যুতে হাড়ে হাড়ে টের পেয়েছি এবং তৎপ্রসূত অভিজ্ঞতা ও নির্মম বাস্তব জ্ঞান আমি ক্রমশঃ আপনাদের সবার সঙ্গে এই প্রবন্ধের মাধ্যমে ভাগাভাগি করতে আগ্রহী।
এক্ষণে, বর্তমান ক্যান্সার চিকিৎসা ব্যবস্থা সম্পর্কে আসা যাক। যদিও এই ক্যান্সার সহ সমস্ত চিকিৎসা প্রক্রিয়ার অহর্নিশি উন্নয়ন ঘটছে বিশেষতঃ আধুনিক কম্পুটার ব্যবস্থার সহযোগে কৃত্রিম বুদ্ধিমত্তার প্রয়োগে অতীতের বহু অসম্ভব কাজ আজ সম্ভব হচ্ছে ও নিখুঁত ভাবে (যেমন সাধারণ নিরপরাধ জনগণের এমনকি ব্যাপক হারে সৈন্যের প্রাণহানি না ঘটিয়েও সঠিক লক্ষ্য ধ্বংস করা যাচ্ছে তেমনই ইন-সিলিকো মলিকুলার ডকিং পদ্ধতিতে, অতীতে সংরক্ষিত তথ্যাবলীর সুষ্ঠু বিশ্লেষণ করে, নূতন সম্ভাব্য ঔষধ আবিস্কার বিষয়ে পরামর্শ দানও কম্পুটারের সাহায্যে সম্ভব হচ্ছে) সম্পাদিত হচ্ছে তবে অ্যালোপ্যাথিক মতে ক্যান্সার চিকিৎসা বড় বড় হাসপাতাল বা নার্সিংহোমেই ও মূলতঃ ব্যয়বহুল কেমোথেরাপিতেই এখনও যেমন সীমাবদ্ধ তেমনই ডাক্তার, হাসপাতাল মালিক-পরিচালক, ঔষধ উৎপাদক ও ব্যবসায়ী এবং বলাই বাহুল্য (প্রত্যক্ষে বা পরোক্ষে) উচ্চ প্রভাবশালী রাজনৈতিক নেতার দুষ্টচক্রের লোভ ও প্রলোভনের খেলা সাধারণ মানুষকে তো বটেই এমনকি অতি বিত্তশালীকেও কিংকর্তব্যবিমূঢ় ও সর্বশ্রান্ত করে অধিকাংশ ক্ষেত্রেই সেই একই বিয়োগান্তক নাটকের যবনিকা ঘটে চলেছে!
অন্যদিকে, হোমিওপ্যাথি, আয়ুর্বেদ, সিদ্ধা, চৈনিক, ইউনানী প্রভৃতি বিকল্প ব্যবস্থায় সাধ্যমত ব্যয়ে ক্যান্সার চিকিৎসা আগেও হত এবং এখনও সম্ভব এবং এই সব চিকিৎসা ব্যবস্থার প্রায় সবই যুগের সাথে তাল রেখে নিজেকে আধুনিকিকরণও করছে। অনন্তর, বিশ্ব স্বাস্থ্য সংস্থার ঐ পরামর্শের সূরেই নিশ্চিত করেই বলা যায় কাঁচা পেঁয়াজ, রসুন, সজনে সহ বহু ফল-মূল ও শাকশব্জী (বিশেষতঃ অ্যান্টি-অক্সিডেন্ট জাতীয় ফাইটোকেমিক্যালগুলি কোষ-বিকৃতি প্রক্রিয়ায় দুষ্ট কোষ সৃষ্টির পূর্বমুহুর্তে লাগাম টানতে সক্ষম এবং অসংখ্য ভেষজ সম্ভারে উপস্থিত বিভিন্ন ফ্লেভোনয়েড ও ফ্লেভোনয়েড গ্লাইকোসাইড জাতীয় ফাইটোকেমিক্যাল ও আরও বিভিন্ন উদ্ভিদ-সঞ্জাত ফাইটোকেমিক্যালের এই গুণ রয়েছে) যেমন কোষ-বিকৃতি রোধ করে ক্যান্সার প্রতিরোধ করতে পারে তেমনই প্রায় সকল উদ্ভিদে বর্তমান (আর এ কারণেই আমরা তাদের রোগমুক্ত নধরদেহ নিরীক্ষণে প্রীত হই!) অ্যালকালয়েড (মূলতঃ মাদাগাস্কারে উৎপন্ন উন্নতমানের নয়নতারা ফুল গাছ থেকে প্রস্তুত ভিনক্রিস্টাইন ও ভিনব্লাস্টাইন অ্যালকালয়েড তো বহুকাল থেকে কেমোথেরাপিতে ব্যবহৃত হচ্ছে, তথ্যসূত্র [৫]), ফাইটোস্টেরোল, টারপিনয়েড, ক্যরোটিনয়েড, ফ্লেভোনয়েড (ও অন্যান্য পলিফেনোল), অ্যাসিটোজেনিন প্রভৃতি ফাইটোকেমিক্যাল কম-বেশি হয় ঐজাতীয় দুষ্ট কোষ ধ্বংসে সক্ষম নাহয় দুষ্ট কোষের অপত্যকোষ সৃষ্টিতে সহায়ক উৎসেচক জাতীয় আমিষ তথা প্রোটিনের কার্যকারিতা রহিত করায় দক্ষ। এ প্রসঙ্গে, অ্যান্নোনাসী পরিবারভূক্ত বিদেশি সাওয়ারসোপ (করোসল, গ্রাভিওলা, গুয়াবানো, গুয়ানাবানা, প-প ইত্যাদি নাম বিভিন্ন দেশে প্রচলিত – আকার কাঁঠালের মত ও ফলের স্বাদ টক-মিষ্টি), আমাদের দেশের আতা (বাঁকুড়া জেলার নাম মান্দার, सीताफल – ফলের স্বাদ মিষ্টি, তথ্যসূত্র [৬]), নোনা আতা (নোনা, रामफल – ফলের স্বাদ নোনতা) প্রভৃতি উদ্ভিদের পাতা, বীজ, ছাল ও শিকড় সহ বিভিন্ন অঙ্গে বর্তমান এবং তেলে দ্রবনীয় ও লম্বা-সৃঙ্খল ফ্যাটি-অ্যাসিডের পরিবর্তিত রূপে জাত অ্যান্নোনাসিয়াস অ্যাসিটোজেনিন জাতীয় ফাইটোকেমিক্যালের কার্যকারিতা যেকোনো কেমোথেরাপিতে ব্যবহৃত ওষুধের চেয়ে কল্পনাতীতভাবে বহগুণে বেশি (অথচ কেমোথেরাপীর আশু মুহুর্মুহু বমি, ডায়রিয়া প্রভৃতি সহ পরবর্তীকালের অসংখ্য খারাপ দিক এড়ানোর সুযোগ রয়েছে), তাছাড়াও এই পরিবারস্থ উদ্ভিদগুলিতে বর্তমান কমবেশি অনেকগুলি অ্যালকালয়েডও ক্যান্সার দমনে পটু – সুদীর্ঘকাল ধরে ও বহু প্রোথিতযশা গবেষকের গবেষণা-প্রসূত ইপ্সিত ফল এই সত্যকে দৃঢ়ভাবে সুপ্রতিষ্ঠিত করেছে! আরও একটু পরিষ্কার হতে এই অ্যান্নোনাসিয়াস অ্যাসিটোজেনিন জাতীয় ফাইটোকেমিক্যালের উদাহরণ স্বরূপ আতা গাছের সর্বাঙ্গ থেকে প্রাপ্তব্য একটি যৌগ Squamocin-কে নিয়ে সাধারণভাবে এদের গঠন ও কার্যকারিতা সম্পর্কে কিছু আলোকপাত করা যাক –
চিত্রে দেখানো ত্রিমাত্রিক আনবিক কাঠামোর মডেলটিতে লক্ষ্যনীয় যে একদিকের মাথায় একটি বালা তথা রিং ও মাঝে কমপক্ষে একটি বালা এজাতীয় যৌগের বৈশিষ্ট (গঠনটি লক্ষ্য করলে সহজেই অনুমেয় যে, এগুলি ফ্যাটি অ্যাসিডেরই পরিবর্তিত রূপ – এই পরিবারের উদ্ভিদগুলির জৈব-রাসায়নিক প্রক্রিয়ায় এই পরিবর্তন ঘটানোর যোগ্যতা রয়েছে) – তবে মাঝে ৩টি পর্যন্ত এই রকম বালা হতে পারে এবং তদনুসারে ও অন্যান্য বৈশিষ্ট (যেমন দৈর্ঘ্য ও এক বা একাধিক হাইড্রক্সিল গ্রুপের অবস্থান ইত্যাদি)-এর জন্য নামের ও কার্যকারিতার ভিন্নতা ঘটে, তবে, মাঝের ঐরূপ রিং তথা বালার সংখ্যা বেশি হলে ক্যান্সার দমনের তেজ বৃদ্ধি পায়। এতদুপলক্ষে উল্লেখ্য যে, (১) যদিও সাধারণভাবে বলা হয় বিষ নিয়মানুসারে ও পরিমিত মাত্রায় ব্যবহারে ঔষধ এবং মাত্রাতিরিক্তে বিপজ্জনক তবে উদ্ভিদ-জাত ফাইটোকেমিক্যালগুলির অধিকাংশেরই পার্শ্বপ্রতিক্রিয়ার ঝুঁকি বেশ কম হলেও স্টেরোয়েড ও অ্যালকালয়েড জাতীয় যৌগ বেশি মাত্রায় ও বারংবার ব্যবহারে ফল খারাপ হওয়ার সম্ভাবনা দেখা গেছে এবং মধ্য-আমেরিকার জনগণ প্রাপ্তির আধিক্যের সুযোগে প্রধান খাদ্য হিসাবে সুদীর্ঘকাল যাবৎ খুব বেশি ঐ সাওয়ারসোপ খাওয়ায় কিছু লোককে পারকিনসন্ নামক রোগের অনুরূপ শারিরিক সমস্যার শিকার হতে লক্ষ্য করা গেছে (তথ্যসূত্র [৭]); (২) একই উদ্ভিদ ও/বা একাধিক উদ্ভিদ থেকে যৌথভাবে কতিপয় ফাইটোকেমিক্যালের সঠিক অনুপাতে মিশ্রণ ব্যবহারে অনেক সময় অধিক সুফল পাওয়া যায় (যেমন দশমূলারিষ্ট, ত্রিফলা ইত্যাদি, যদিও এক্ষেত্রে বিশেষ বিশেষ যৌগ নিষ্কাশনপূর্বক মেশান হয় না) এবং (৩) সযত্নে মনে রাখা একান্ত জরুরী যে, কোন ঔষধ ব্যবহারের আগে বিশেষজ্ঞের পরামর্শ মাফিক মাত্রা ও ব্যবহারবিধি মেনে চলা উচিৎ।
সবই তো ঠিকই চলত, যদি সমাজে সর্বস্তরে ন্যায় ও ভারসাম্য বিরাজ করত। আগের আলোচনাটি একটি সহজ-সরল শিশু যদি হৃদয়ংগম করত তাহলে, স্বাভাবিকভাবেই তার মনে প্রশ্ন জাগত – যদি ঐ অ্যান্নোনাসিয়াস অ্যাসিটোজেনিন জাতীয় ফাইটোকেমিক্যালগুলির কার্যকারিতা এতই বেশি, ঔষধরূপে অর্থাৎ পরিমিত মাত্রায় পার্শ্বপ্রতিক্রিয়া মুক্ত ও সহজ লভ্য (সর্বাধিক শক্তিশালী সাওয়ারসোপের আদি নিবাস আমেরিকা যুক্তরাষ্ট্রের দক্ষিণ, মধ্য আমেরিকার দেশগুলি ও দক্ষিণ আমেরিকার উত্তরাঞ্চল হলেও বাংলাদেশ সহ বিভিন্ন দেশেও বর্তমানে চাষ হচ্ছে তবে ঐ পরিবারস্থ অন্যান্য বহু প্রজাতী কম-বেশি প্রায় সব দেশে সহজ লভ্য) হয় তাহলে সেগুলির প্রয়োগ চালু হচ্ছে না কেন? উত্তর সেই একটাই – ঐ দুষ্টচক্র চায় না যে এমত মারণ রোগ সহজে ও স্বল্প খরচে তাদের বিশ্বব্যাপি মুনাফাবাজী টপকে সমাধান হয়ে যাক এবং তদনুসারে ডাক্তারবাবুদেরকেউ সযত্নে ওয়াকিবহাল করে দেওয়া হচ্ছে – অনন্তর, পৃথিবীব্যাপি সুফলদায়ী ও যুগান্তকারী গবেষণাগুলি মাঠে মারা যাচ্ছে!
